We have received funding to deliver a new service across West London - Homeless Prevention service and we are looking for a highly motivated individual to join this new team and help to shape the service.
In conjunction with other members of the team, you will deliver a rapid assessment service to assess people at risk of rough sleeping, either by phone or email – in partnership with, day centres, outreach teams, local authority and other partners. You will also be the first point of contact for the Team in relation to face to face, telephone and email communication. To answer and monitor outreach referrals, including Streetlink. To develop and maintain paper and electronic data systems and produce statistical and written reports as directed
To succeed, you will have the ability to work as part of a team and to initiate and maintain effective working relationships with others; including experience of liaison responsibility in a multi-agency service environment. You will also have strong IT/typing skills and good practical knowledge of Microsoft Office and accounting/petty cash procedures. Other requirements are the ability to handle responsibility and manage your time and work in demanding and target driven role.
