About the role

HOPE – Helping Older People Engage

A partnership of Caledonia, Hillcrest and Dundee Health & Social Care Partnership has evolved to support and promote independent living of older people in communities across the city. HOPE's main aim is to help older people feel less lonely and isolated by encouraging participation in local groups and activities as well as providing advice and support on a range of issues such as local services, housing or care options.

This is a rewarding role where the Community Engagement Worker will provide a visiting service to older people in their local community that assists with life transitions such as bereavement, social isolation, moving house or following periods of ill health.

Support workers will help people over the age of 55 connect with services and activities in their local communities as well as offering practical assistance, sign posting and referrals to more specialist services, such as welfare and benefit checks, occupational therapy aids and adaptations to help individuals live at home independently.

If you have previous experience of supporting the needs of older people, and are passionate about delivering an outcomes oriented, innovative, individually designed, and needs led, support service, we would like to hear from you.
