Outreach Case Worker Reading Fixed-term contract until June 2019

We are looking for a highly motivated individual to join our dynamic street outreach team in Reading. We work across the city delivering shifts on the streets, drop-ins in local day centres, and work with accommodation providers and the local authority to reduce rough sleeping.
As an Outreach Worker you will be involved in making contact with all rough sleepers and those at risk of homelessness; assessing their needs and delivering personalised case management to support them to make the move away from the streets and/or street lifestyles for good. This will involve liaising with key services throughout the UK and Europe.
To be successful, you will have experience of working with vulnerable adults in a social care, health, community safety or housing setting. You will also have experience of multi-disciplinary agency working and the necessary social skills to initiate and maintain constructive and appropriate relations with clients and agencies. The ability to drive a car and hold a clean driving license is also essential.

Flexibility is required as you will be working shift rota hours, mainly a combination of early (5am - 1pm, 6am - 2pm, 7am - 3pm) and day shifts with occasional late shifts; to best meet clients on the streets.
